  • Frequency of multimorbidity and polypharmacy among geriatric patients

    Hungary is characterized by an ageing society. The majority of healthcare is used by the elderly. Hospitalization is frequently required due to iatrogenesis, caused by drug toxicity and adverse interactions. Polypharmacy can augment these problems. 
    The purpose of my research is to assess the frequency and harmful effects of polypharmacy in an elderly inpatient care unit. In addition to the survey results, during the research, which was conducted in an inpatient department, I collected data on regularly taken medications, the established diagnoses as well as the results of kidney function test. Informed consent was obtained from each patient. 
    According to the research, it was proven that among geriatric patients, polypharmacy is an important contributor to their potential risk. The more diseases a person suffers from, the more medications he or she must take, so more drug interactions must be expected. Therefore, it is extremely important to regularly reconsider databases concerning drug interactions. To improve patient safety, the "unit-dose system" provides great help concerning inpatients, which also includes the automatic monitoring of interactions. With the help of all this, the development of complications, hospitalization and mortality can be reduced. Decreasing iatrogenicity not only improves the patient's quality of life but also lowers the burden on the healthcare system.

Based on the complex examination of diagnosis, prognosis and rehabilitation, the primary consideration for individuals who cannot be rehabilitated is to maintain their independence as long as possible, which goes hand in hand with a better quality of life. A kutatás során a University Hospitals of Leuven pellenbergi campusán fejlesztett és vizsgált Geriatric Activation Program Pellenberg (GAPP) (Van Dijk et al., 2021) struktúrált, funkcionális rehabilitációs programjának adaptálása történik a Semmelweis Egyetem Geriátriai Klinika és Ápolástudományi Központ betegeire. A kutatás célja a geriátriai betegek komplex fizioterápiás rehabilitációja, valamint a funkcionális teljesítmény alakulásának vizsgálata és a kimutatható változások előrejelzése a vizsgált időszakban.

    Purpose: This study aimed to compare the sociodemographic characteristics in nursing home residents and community-dwelling elders.

    Methods: This descriptive study consisted of 342 older people between October and November 2019 (nursing home = 79, community-dwelling = 263). The data were collected by “Questionnaire Form for Individuals Living in Nursing Homes” and “Questionnaires for Individuals Living in Community-Dwelling." Comparisons between groups were made with the chi-square test. Descriptive characteristics were presented as numbers, percentage, mean, standard deviation, min. and max. scores.

    Results: The average age of the elderly living in a nursing home was 77.35±7.40 (min: 66, max: 97). Before coming to the nursing home, 38.0% lived with their spouses. The average age of community-dwelling older people was 70.90±5.57 (min: 65, max: 88). Of community-dwelling older people, 81.7% lived with their families. There was a significant difference between both groups regarding age, income status, having a child, having a physical disability, using dentures, ability to maintain activities of daily living, self-confidence, sleep problems, social activity, smoking rates, and history of visiting a nursing home (p < 0.05).

    Conclusion: Compared to the community-dwelling older people, the elderly living in a nursing home were more senior, lonely, had a lower income, had higher physical disabilities, had lower social activity and self-confidence, had more sleep problems, and smoked.

    Purpose:
    This study was conducted as a descriptive and comparative study to determine why a nursing home was chosen for the elderly.

    Methods:
    The population of the descriptive study consisted of older people living in a community-dwelling or a nursing home in the Central Anatolian Region of Turkey. The sample consisted of 342 elderly individuals who agreed to participate in the study between October and November 2019 (nursing home: n = 79, community-dwelling: n = 263). After obtaining the permission of the ethics committee (no: 2019/04-03), the data were collected by “Questionnaire Form for Individuals Living in Nursing Homes” and “Questionnaires for Individuals Living in Community-Dwelling."
    The Chi-square Test analyzed data, and the descriptive characteristics were presented as numbers, percentage, mean, standard deviation, minimum and maximum.

    Results:
    The average age of the elderly living in a nursing home was 77.35±7.40 (min: 66, max: 97), and the average age of community-dwelling older people was 70.90±5.57 (min: 65, max: 88). Before coming to the nursing home, 38.0% lived with their spouses, and 35.4% lived alone. 81.7% of community-dwelling older people lived with their families. There was a difference in terms of some sociodemographic variables between both groups. Elderly people staying in nursing home; 26.6% were in the 76-81 age group; 26.6% did not have children; 38.0% had no income; 16.5% had a physical disability; 82.3% used dentures; 13.9% couldn’t do their daily care; 22.8% had low self-confident; 62.0% had sleep problems; 67.1% had no social activity; 35.4% were smokers; 88.6% of them had not visited a nursing home before (p < 0.05).

    Conclusion:
    Compared to the community-dwelling older people, the elderly living in a nursing home were more senior, lonely, had a lower income, had higher physical disabilities, had lower social activity and self-confidence, had more sleep problems, and smoked.

    The ageing is the global phenomenon, it is main more difficult financial and social
    problem for modern societies. If we accepted the ageing = disease identity, this does not
    help solving the problem, it increases cost only. It is still high number of people over the
    age of 65 in hospital inpatient departments. A change of view is needed. The aging is
    regarded as a decompensation process, which has parts and interventions possibilities. If
    we intervene in the downturns of the decompensation process with appropriate means,
    decompensation can be reduced; life-years in health can be increased. The goal is to
    preserve self-sufficiency as much as possible. Should be system established, because in
    other way this will be for profit service only. We have reviewed the major experiments that
    have taken place in the world and seem appropriate to handle the issue properly. However
    in order to achieve results, necessary change not only the structure bat also the attitudes.

    According to Botswana's Current statistics for 2021, people aged 60 and above would make up 8.9% of the population. As people age, they face a plethora of challenges; mental, legal, social, health, economic, environmental, and political, in addition to ageism and marginalization. Therefore, they need resilience to deal with these challenges that emanate from the aging process, the development of care needs, and the depletion of resources. However, in Africa, there is anecdotal evidence that some older people cannot cope with their lives, let alone carer responsibilities caused by HIV related death and other risk factors. For older people to flourish, they need resilience to achieve, endure, develop and sustain their health and well-being in the face of adversity. The degree of success and impact on their resilience is undocumented. The research will therefore determine the correlates with resilience and establish ways to curb the risk factors.

    A körülöttünk lévő természet elláthat minket olyan gyógyító növényekkel, amelyek segítségével könnyebben tudjuk átvészelni és kezelni a mindennapi problémákat, betegségeket. A gyógynövények jelenleg ismét reneszánszukat élik, és egyre többen mernek hozzájuk fordulni egészségük megőrzése érdekében (Bernáth, Németh, 2007).

    Idősebb korban még nagyobb hasznukat vehetjük, hiszen génjeink, környezetünk vagy életvitelünk következményeként ekkor számos betegség felütheti fejét, melyek ellen a küzdelem már nem olyan egyszerű egy 60 év feletti szervezet számára. Manapság egyre jobban növekszik az idősek között a szív és érrendszeri, valamint a mozgásszervi megbetegedések száma, melyek a vezető megbetegedési csoportok között találhatóak ebben a korosztályban (http://www.ksh.hu).

    A testnek ekkor már nehezebben működnek az életfolyamatai, ezek könnyen eltolódhatnak a rossz irányba, az immunrendszer sem működik teljes kapacitással és akár egy kisebb betegség is hosszútávon kihathat az egészségre.

    A gyógynövények segítségével számos betegség megelőzhető, kezelhető vagy éppen segítségükkel az egészség helyreállítható. Csupán körül kell néznünk a minket körülvevő természetben és megtalálhatjuk a számunkra megfelelő növényt.

    Hazánk rendkívül jó természeti adottságai miatt bővelkedünk gyógynövényekben, körülbelül 400 féle található nálunk, mely egész Európa szerte igen jelentős mennyiségnek számít. Egy-egy növény igazán komplex gyógyító hatással rendelkezik és akár többféle panasz kezelésére is használható. Megfelelő körültekintés mellett alkalmazva pedig nagy biztonsággal és igazán jó eredményekkel alkalmazhatóak. Magyarország természeti adottságai lehetővé teszik, hogy számos gyógynövény közül válogathassunk és segítségül hívjuk őket betegségek megelőzéséhez vagy éppen gyógyításához. Kiegészítő terápiaként nagyon hasznosak lehetnek akár az időskori megbetegedések esetén is (Bernáth, Németh, 2007).

    Cél és módszer:
    A kutatásban szerettük volna feltérképezni az időskori panaszok és betegségek, illetve a gyógynövények használatának, valamint az egészséges életmódnak a kapcsolatát és a 60 év felettiek gyógynövényekről való véleményét, melyet saját összeállítású, internetes kérdőív segített, melyet 123-an töltöttek ki.

    Eredmények:
    A kérdőívre érkezett válaszok alapján a válaszadók, ha tehetik, szívesen alkalmazzák a gyógynövényeket, sokat ismernek közülük és megbíznak azok jótékony hatásában, többek között azért, mert elég sok pozitív tapasztalatuk volt már használatukkal kapcsolatban.

    Egyik, kérdőívben feltett kérdésünkre érkezett válaszok alapján az alábbi gyógynövényeket alkalmazzák leggyakrabban az idős emberek a házi patikában: galagonya, citromfű, kakukkfű, fekete nadálytő, borsmenta, csalán, levendula, zsálya, kamilla, körömvirág, gyömbér, csipkebogyó, útifű, dió és a cickafark.

    Leggyakrabban teák, étrend kiegészítő tabletták, kapszulák formájában használhatóak fel ezek a növények, de készíthető belőlük gyógynövényes fürdő, olajos és alkoholos kivonat, valamint kenőcs is akár, illetve nagy részük ételek ízesítésére is használható vagy akár salátaként is fogyasztható.

    Egytől egyig mindegyik felsorolt növény igen hasznos lehet az idősebb korosztály számára is, de rajtuk kívül még más növényeket is érdemes ismerni és alkalmazni. A biztonságos gyógynövény használathoz tudni kell, hogy mire, milyen formában és mennyi ideig alkalmazhatóak, illetve, hogy rendelkeznek e esetleg valamilyen mellékhatással, ellenjavallattal vagy gyógyszerekkel való kölcsönhatással (Szendrei, Csupor, 2009).

    Az egészséges emberek tudnak dolgozni, eredményeket létrehozni, teljes életet élni és legtöbbet tenni a társadalomért. Az egészséges életmódra nevelést, a népegészségügyi programokat már fiatal korban el kellene kezdeni, beleszőve az átadott tudásanyagba a komplementer medicina lehetőségeit, többek között a gyógynövények ismeretét is. A gyógynövények lehetőséget adnak arra, hogy az emberek segítsenek magukon, és megtanulják a saját kompetenciahatáraikat az öngyógyítás területén.

    Azonban a tapasztalat azt mutatja, hogy nagyon sok ismeretterjesztő programra, előadásra lenne szükség ahhoz, hogy a tévhiteket eloszlassuk és biztonságosabbá tegyük a házi patikákban is a gyógynövények használatát.

    A kutatásban bemutatjuk az idősek számára ajánlott legfontosabb hazai és külföldi gyógynövényeket, amelyek megfelelő körültekintés mellett biztonságosan alkalmazhatóak.

Az idős betegek ellátása az egészségügyi költségvetés több, mint 30%-át teszi ki. A geriátriai populációt multimorbiditás és polipragmázia, funkcionális függőség, esendőség és depresszió jellemzi, mely speciális tudást igényel mind az orvosok, mind a szakdolgozók részéről.

    Background and aims: In our research, we examined how the last stage of their lives is experienced by older people who engage in creative activity (handicraft, fine arts, textile art) aimed at creating a lasting work at this stage of their life.
    Methods: In our study, we used questionnaires examining proactive coping, life satisfaction, geriatric depression, and the meaning of life.
    Results and discussion: The higher level of search for meaning measured by creators may be related to the complex nature of the search, which may involve the mental stress associated with life cycle change and the ability to cope with it and the possibility of development, so that creative activity can be interpreted as a kind of protective factor. In the group of creators, reflective coping (exploring and analyzing several solution options and the resources needed for the solution) showed the highest value, which may result from the creativity and divergent thinking of those engaged in creative activity.

    Dear Reader!
    The regular issue of Hungarian Gerontology 2022 contains 13 interesting articles on different areas of gerontology and geriatrics. Sveral publications deal with the current issues of ageing and digitalisation and robotic technologies, one study deals with new directions in geriatric rehabilitation, but the reader can also find empirical results on the psychological aspects of successful ageing or the values of the Roma population in relation to elderly care. The importance of writing in maintaining activity in old age is also discussed, as well as film analysis, book reviews, and reviews of scientific events.

    We would also like to draw the reader's attention to our yearly conference in 2023, with presentations in Hungarian on 12 October and in English on 13 October. We look forward to welcoming you as a speaker or participant on Gerontology Days 2023 International Scientific Conference!
