  • Quality of life for social care centre clients

    Introduction:
    There are approximately 60 million people over 60 years of age in the world. The United Nations predicts that by 2050 the number of people aged 60 years and older could be around 2 billion (Zaļkalns, 2015). In Latvia, every fourth inhabitant is a pensioner. Given Latvia's socio-economic situation, social and health care options for older, lonely people are becoming increasingly less accessible. A large part of the population of Latvia cannot provide themselves with quality of life in old age. If there is no family or, due to various circumstances, relatives cannot take care of the elderly, the only option is social care institutions (Slokenbeka, Zepa, 2013). The quality of life of an elderly person remains an issue when he or she is in a social care centre. Quality of life is a complex, interacting set of objective and subjective indicators in different areas of life. The World Health Organisation defines quality of life as “the perception of an individual's personal life position in the context of the cultural and value system in which the individual lives in relation to the individual's goals, expectations, standards and concerns. It is a broad concept that is influenced in complex ways by a person's physical health, psychological state, personal beliefs, social relationships and key environmental factors” (Scester, 2012).

    The Aim of the study:
    To analyse the quality of life for social care centre clients.

    Materials and methods:
    Quantitative research method was chosen to obtain the results. A questionnaire with 25 questions was developed. The participants of the research were clients of two social care centres (hereafter SAC) (SAC “X”, n = 50; SAC “Y”, n = 50).

    Results:
    The majority of SAC respondents have lived in the institution for more than four years (SAC “X” = 54%; SAC “Y” = 70%). In both groups, the predominant reason for being in SAC is “I am lonely, I cannot take care of myself” (SAC "X" = 62%; SAC "Y" = 58%). On the questions about the frequency of meals and the quality of food in the SAC, the data show that 78% of clients in SAC “X” and 94% in SAC “Y” are satisfied with the frequency of meals, while on the quality of food the dominant answer is “the food is satisfactory” (SAC “X” = 54%; SAC “Y” = 32%). The questions on living conditions show that SAC clients live both alone and in pairs (mainly spouses). The relationship with the roommate is described by 52% in SAC “X” and 38% in SAC “Y” with the phrase “we get along peacefully, without quarrels”. 26% of respondents in both groups describe their relationship as “very good and friendly”. SAC clients describe their financial situation as “modest”. Emotional support is received from other residents, staff and family members/relatives. Emotional uplift is also provided by various activities in the SAC and by doing things that they enjoy and find interesting, e.g., handicrafts, crossword puzzles. SAC clients note that they try to attend all events organised by the SAC, especially concerts by amateur groups and famous artists. The “feeling of security” in the SAC is prevalent in both groups of respondents (SAC “X” = 56%; SAC “Y” = 70%). Although there is a feeling of security, both groups of respondents report that they “miss the feeling of home” when living in SAC (SAC “X” = 24%; SAC “Y” = 34%).

    Conclusions:
    Clients in the social care centre are satisfied with the quality of life in the physical and social spheres, but are partially satisfied or dissatisfied with the quality of life in the emotional and area of independence. The participants often feel lonely and sad and experience longing and anticipation. Respondents in both social care centres never or rarely experience feelings such as love, joy and happiness. Respondents indicate a lack of independence, acknowledging that they have limited autonomy and that they cannot be who they are because they have to adapt to the existing regime.

  • COVID-19, lockdown, elderly. Experiences of the follow up research among active older adults 2020-2021.

    Introduction, aims:
    The COVID-19 pandemic caused invaluable harms on the World during last two years. Among all economic and societal consequences the negative effects for elderly people was also apparent. Researches - among others - focused on the age related inequalities for accessing and using services, economic disadvantages, the decline of psychical, physical and mental conditions, reduced quality of life, increased level of loneliness, stress and depression, such as the increase and new faces of ageism.
    This research focused on the individual and organizational consequences of COVID – 19 pandemic and the effects of the state interventions followed by. The target group were identified as active seniors who have leading positions in different local and regional senior’s associations. These people were affected individually as a member of the age group and as an experts with organizational responsibilities as well.

    Research method:
    We chose a qualitative follow up (longitudinal) research method that was committed by semi structured phone interviews, recorded and anonymised. The time of the two data record focused and followed the main Hungarian waves of the pandemic: May – June in 2020 and June – July in 2021. Sample were collected from all districts of Hungary (n=42).

    Main research topics were:
    - The situation of the older adults (experiences about the local older people, daily life, problems, issues, social connections)
    - The life in the organization (activities, new initiatives, problems, issues)
    - Individual experiences (fears, daily life, social connections, shopping habits, use of ICT, vaccination).

    Results:
    We found significant differences on the personal life situation and the perception of pandemic and related interventions during the two waves. Pandemic situation may influenced these results. The age related “stay at home campaign” made more difficulties for the older people during the first wave, meanwhile related interventions weren’t so strict during the second wave by introducing shopping timeline for older people and night curfew for all in the late nights. Even first wave did not cause such harm like the second one in Hungary.
    During the first wave we found older people as a rule following citizens. They followed all the restrictions and regulations strictly. Life situations mainly were determined by the living conditions, that is means the ones who lived in the countryside mainly a house with garden experienced less negative effects than those, who lived in the housing estate area (first wave mainly fall on March – May).
    Older people reported increased importance of the local authorities. With the lack of central supporting interventions, local authorities played main role for local support, care and security. If the local government took care of older people they felt safety. Less interventions and coordination made older people insecured.
    The second wave made new situations for people. Older adults became one of the first target groups in vaccination and people started to cope with the pandemic.
    Older adults became more critical with governmental interventions.
    Critics focussed on the
    - pandemic related communication, the vaccination (older people mainly got Shinofarm vaccine that was not accepted in the EU at that time),
    - difficulties with the availability of health services, and
    - they experienced increased economic problems.
    The life in the organization: during the first wave we found frozen life of the seniors organization, cancelled and delayed programmes. Some initiatives were also reported. People preferred to keep contact via phone instead of using another ICT tools. Some of the answerer worried about the community life, how they can restart after the pandemic, others preferred the forthcoming chance for personal meetings.
    Seniors organisations may play an important role of the senior’s life by organizing free time and social activities, advocacy, and many other aspects of active ageing. As we made a first extract of this research further analyse will focus more on the good examples and new initiatives on the social, community and organizational levels.

    Az utóbbi évek eseményeinek hatására egyre inkább felértékelődik az egészség, s annak megőrzése. Az egészség tényezői egyben életmódunk elemei, az egészséges életmód kialakításában pedig az egyénnek kiemelten nagy szerepe van. Az életmóddal összefüggő betegségek (civilizációs betegségek) ma Európában az elsőszámú halálozási okokat jelentik. Jelen tanulmány fő témaköre a várható és a lehetséges életminőség és élettartam rejtélye. Szakirodalmi feldolgozás során választ kerestünk arra a kérdésre, hogy melyek az egészséget, életmódot leginkább befolyásoló tényezők. A Központi Statisztikai Hivatal (KSH) adatfeldolgozása alapján feldolgoztuk a magyar lakosság általános egészségi állapotát vizsgáló egészségügyi statisztikák eredményeit. Ezt követően ismertetjük az egészség pilléreit és szerepüket az egészséges életmód kialakításában, valamint az egészség pilléreinek életmódba való beépítését ösztönző egészségturisztikai szolgáltatásokat az idős generáció számára.

  • University of the Third Age at the Faculty of Health and Social Sciences, the University of South Bohemia

    The goal of the paper is to present almost 30 years of activities of the University of the Third Age at the Faculty of Health and Social Sciences, the history of which started in 1992.
    The first discipline called Care of Humans and their Health was opened by the University of South Bohemia in the academic year 1992/93. The elderly were offered an educational six-term health and social program, while three hours of direct lessons and two counseling sessions a week (“senior Thursday) were subsidized. The students had to sit for unmarked and marked examinations, and to process and defend a thesis at the end of their study. In July 1995, 28 graduates were awarded a certificate on an extramural education in a festive ceremony. The implementation of the discipline of Care of Humans and their Health confirmed that even a non—professional leisure-time education could be provided at an “academic” level. A comprehensive view of education enabled to identify areas that should be taken into consideration during the education of elderly adults. They include areas relating to computer and functional literacy, cultivation of leisure time, culture of the interpersonal mutuality, and the human being (looking for the sense of life and higher goals).
    In the course of the thirty-year history of the University of the Third Age many activities have developed and the conception has extended. The new educational subject called Man in Health, Disease and Distress was transformed into a two-stage program offering the choice of the length and type of the educational program. The option A represented the choice of an integral and topically closed discipline while the options B and C represented the choice of the educational demandingness and inclusion in the European educational program called SoLiLL: Self-Organized Learning in Later Life. Furthermore, a conception of an extending program called Quality of life in the Old Age and other programs were offered to the elderly living in our region. As a part of the project activities, the elderly were involved in the projects of Phare a Interreg IIIA. The conception of the University of the Third Age at the Faculty of Health and Social Sciences of the University of South Bohemia enables the applicants to complete an integral educational program, lecture blocks/cycles with various topics, and, at the same time, a unique research program called “Golden Path” focused on the Czech-Austrian border area can be completed. The uniqueness of the educational research program is evidenced by two translated monographs called “Wallern und Wallerer (Volary a Volarští - Volary citizens and Volary)“ and „Der Goldene Steig“ (Zlatá stezka – Golden Path) by the indisputably most important expert and researcher of the Golden Path, a historian living in Bavarian Waldkirchen.

  • Palliative and Hospice Social Work Roles in the U.S.

    Palliative care services worldwide continue to grow, primarily in response to a human rights approach and to respond to the aging of the population, increasing prevalence of chronic illnesses and cancer mortality. While there is recognition in the WHO definition of palliative care that not only physical, but also psychological, social and spiritual aspects of care must be part of services provided, how these are addressed varies greatly by country and region of the world.
    In the U.S., social work services are mandated to be provided by hospice organizations seeking funding from Medicare (governmental insurance for people over age 65) and supports the tenets of the palliative care philosophy to provide person-centered holistic care. The role of palliative and hospice social workers is highly aligned with the values of the profession centered on the dignity and worth of all humans and the commitment to improving quality of life throughout life and especially at the end of life. Older adults make up the overwhelming majority of hospice recipients in the U.S. and attention to their daily needs and those of the family caregivers is essential to maximize quality of life.
    This presentation will focus on the roles of social workers in the U.S., particularly with older adults, in various palliative care settings and how this may compare to roles across the globe.

  • Az idősek és az infokommunikációs technológiák elsődleges vonatkozásai

    Cikkünkben kitérünk az információs és kommunikációs technológia (továbbiakban IKT) rövid áttekintésére, az idős korosztály legjellemzőbb sajátosságaira, végül azt vizsgáljuk, hogyan és mely részterületeken tudnak kapcsolódni az idősek az IKT megoldásaihoz. A gyorsan fejlődő technológia világában még azoknak sem könnyű kiigazodni, akiknek nem kell gondolkozni azon, hogy hogyan tanulják meg az IKT használatát. Komoly hátránnyal indulnak azok, akik életük egyharmadában, felében nem voltak kapcsolatban az infokommunikációs technológiákkal, idős korban pedig ez a hátrány tovább növekedhet. Manapság egyre nehezebb különbséget tenni online és offline világ között és valószínűleg ez a jövőben sem lesz könnyebb. A jóléti államokat elöregedő társadalom jellemzi, az ezekből fakadó problémák régóta jelen vannak minden területen, ahogy az egészségügyben, úgy a gazdaságban is. Tanulmányunkban megvizsgáljuk és összegyűjtjük a témakörhöz szorosan kapcsolódó releváns szakirodalmakat, melyek az életminőségre, a magányosság leküzdésére, a társadalmi felzárkóztatásra és az egyén egészségi állapotára vonatkoznak. Célunk, hogy megvizsgáljuk milyen lehetőségekkel segíti az IKT az idős emberek minőségi öregedését, milyen tényezők motiválják az időseket az eszközök használatában, melyek azok a nehézségek, amelyek akadályozzák az alkalmazást és a tanulást, illetve, hogy összességében milyen jövőbeni kilátások elé nézünk. Vizsgálatunk angol és magyar nyelvű szakirodalomra vonatkozott, azokra a publikációkra, melyek Európában jelentek meg vagy európai országokban végezték a kutatást, a 65 éves vagy idősebb emberek körében, 2011 és 2021 között.

  • Ageless quarantine yoga - Escape to Yogaland in the time of pandemic

    Nowadays it is expansively recognised that practicing yoga can improve the quality of life by providing appropriate physical training exercises which can be performed by every age group. Yoga practitioners of the older generations beside maintaining their physical activity can heal  their sense of balance which decreases the hazard of off-balance and fall.

    During the years of my yoga teaching I met the representatives of every age group at my classes. I led kids yoga courses for preschool children, dynamic flow sequences for trained yoga practitioners, but I did have students over 70 years old who insisted to join an intermediate level hatha yoga class even if they had gone through serious illnesses and operations in the past and they were not able to hold each postures.

    I have experienced that the elderly people are very grateful for the healing that yoga gives them and for the careful attention whereby the yoga instructor tailors the yoga asanas and sequences according to the physical condtitions, state of health and capabilities of each individual.

    At the yoga instructor courses the contraindications of asanas and defining the anatomic and physiological backround of injuries and the physical limitations of each practitioner are emphasized intensively. This comprehensive teacher training and the continuous monitoring guarantee the safety. The instructor faces the mutations and the loss of physical and psychical balance in the the reality, at the yoga classes. Practice makes the master! It is more than true is yoga. Gaining practical experience the instructor can handle the special situations and needs with growing confidence applying yoga props (strap, yoga blocks, chair etc.), modifying the asanas, and using the power of words to motivate the students.

    Supported by my friends I created The Force Yoga Group Facebook site in April , where I have been leading yoga classes since then. It is a great place to meet my yoga practitioning friends, my family members, my elderly parents and in defieance of quarantine and lockdown to practice yoga together, at the same time. Our magical yoga carpet is a tranquil island to where we can escape from the raging pandemic, where we can start our inner journey in the time of the outer movelessness.

    The online practice team has already more than 100 members. It is an intercultural and intergenerational group. I knew I had no information of the health conditions, the perfection of yoga practice of each group member, and with many of them I had not practiced together in person (offline), so I had to call their attention to the rules of safe yoga practice constantly and acutely. At the beginning and at the end of the online sessions I dedicate some sentences to it, during practice I try to instruct precisely to protect them from the injuries. The verbal correction playes here a very important role.

    My online students can be informed about the topic from the scientific articles I share on our page from time to time.

    I am aware of the fact that an avarege home is not a well-equipped yoga studio, so  I show online how to use the furniture, fixtures and everyday objects as yoga props.

    A shorter, 30-45 minute Chair Yoga Class is the part of our weekly program as well, especially for those members who struggle with balance poses or get weak easily.

    In private messages the students send me their observations, remarks, questions  and requests. Some of them experience pain or tension in certain asanas, so we try to find out together the reason of it and to correct and to set the posture. A 75 year-old student of mine asked me to build a yoga sequence of simple breathing exercises and stretching asanas which can be performed by anyone.

    Many feedbacks speak about how big inspiration is to watch my everyday practice and my enthusiasm is pushing the spectators to their yoga mat. There are older practitioners who perform the breathing and warm up exercises sitting on a chair, and it means already 15-20 minutes of  physical training.

    During lockdown there are 4-5 online yoga sessions weekly, the videos are available anytime. The regular practice becomes a sure point in the life of the members, there is something to look forward to, even if there is no stimulus to receive from the outer world. In this online yoga space you can get new friends, through the comments you can share your ideas and suddenly you belong to a yoga community.

    The stress relieving classes I close with  Yoga Dream (yoga nidra) relaxation which is an at least 20 minutes of visualization, an imaginery journey interlarded by positive affirmations while body and mind are calming down.  Such a mini meditation does not require any intensive physical activity but breathing. I could mention many other positive increments that occured creating The Force Yoga Group, but quoted the great yoga master, B.K.S. Iyengar:

     “Words cannot convey the value of yoga – it has to be experienced.”

  • Időskori életminőség, jóllét és idősödéssel kapcsolatos attitűdök vizsgálata

    Az idősödés folyamatának megélése számos tényezőtől függ, ilyen például az egészségi állapot, a társas kapcsolatok, az életünk során kitűzött célok teljesítése, életünk értelmének beteljesítése, az élettel való elégedettség, a jóllét vagy az életminőség. Az idősödés veszteség modellje szerint az idősödés negatív folyamatként, veszteségek sorozataként értelmezhető. (Lampek és Rétsági, 2015) Ezzel szemben a sikeres idősödés pedig új erőforrások kialakítására képes viselkedésmintázatnak tekinthető, magában hordozza a rugalmasságot, pozitív életszemléletet és a személyes fejlődés lehetőségét egyaránt (Baltes, Smith és Staudinger, 2000; Kerekes, 2013). A jóllét konstruktuma számos tényezőt foglal magában, úgy az általános jóllétet (fizikai, lelki, szociális jólléthez szükséges feltételek megléte), mint a szubjektív jóllétet (egyén érzése, hogy a feltételek mennyire elégítik ki saját belső igényeit) vagy az érzelmeket. Az életminőség magában foglalja a fizikális állapotot, testi jóllétet, a pszichés állapotot, a szubjektív jóllétet, valamint a szociális tevékenységet a társadalomban. Az életminőség és a jóllét fogalmakat gyakran azonos értelemben is használják. (Koós, 2018; Kopp és Martos, 2011; Szántó és mts., 2016)

    Számos kérdőíves lehetőség van a jóllét és az életminőség felmérésére. Kérdéses, hogy az idősekkel kapcsolatos kutatások során mely kérdőíveket célszerű használni, melyek szolgálhatnak többletinformációval vagy épp melyek mérnek nagyon hasonló konstrutumokat. Két kutatásban többek között az idősödéssel kapcsolatos attitűdök, valamint a jóllét és az
    időskori életminőség kapcsolatát vizsgáltuk, illetve azt, hogy a jóllétet mérő kérdőívek közül melyik alkalmazása lehet a legcélravezetőbb.

    1. Kutatás
    Az 1. kutatásban (N=110; 24 férfi és 86 nő; életkor: 65-97 év; önellátók és idősotthonban élők; papír alapú kérdőív) többek között az idősödéssel kapcsolatos attitűdök (AAQ – Attitudes to Agening Questionnaire, magyar nyelvű validáció: Tróznai és Kullmann, 2007), az élettel való elégedettség (SWLS – Satisfaction With Life Scale, magyar nyelvű validáció: Martos és mts., 2014) és az életstílus elégedettség (LSS – Lifestyle Satisfaction Scale, Cambell A, Converse PE. és Rodgers WL, 1976) mérésére szolgáló kérdőíveket használtuk. Az élettel való elégedettség és az életstílus elégedettség egy adott személy életminőségének szubjektív kognitív értékelése, a pozitív életminőség, a lelki egészség, szubjektív jóllét összetevője. Az idősödéssel kapcsolatos attitűdök mérésére kidolgozott kérdőív három alskálát mér: a pszichológiai növekedés alskála az egész életen át tartó fejlődés lehetőségét fogalmazza meg, az élet pozitívumaira kérdez rá, a fizikai változás alskála az időskorban bekövetkező testi működésre, annak változásaira kérdez rá, míg a pszichoszociális veszteség alskála a pszichés és szociális veszteségeket foglalja magában, az idősödésre, mint negatív élményre kérdez rá.

    Eredmények
    Az eredmények alapján megállapítható, hogy mind az élettel való elégedettség, mind pedig az életstílus elégedettség esetében hasonló mértékű és irányú kapcsolat van az idősödéssel kapcsolatos attitűdök alskáláival, magasabb fokú elégedettség és jóllét esetén inkább növekedésként élik meg az idősödést. Az életstílus elégedettség skála túlzottan magas belső konzisztenciája arra utal, hogy az idős válaszadóknál nem különültek el az egyes dimenziók, a kérdéseket nagyon hasonlónak ítélték meg a válaszadók, így az egyes dimenziók nem szolgálnak többletinformációval idős válaszadók esetében. Ezen kívül az élettel való elégedettség skálával magas korrelációja miatt nem érdemes mindkét kérdőívet használni egy vizsgálaton belül.

    2. Kutatás
    A 2. kutatásban (N=450 fő; 174 férfi és 276 nő; életkor: 60-91 év; önellátók; papír alapú és online kérdőív) az idősödéssel kapcsolatos attitűdök (AAQ) és az élettel való elégedettség (SWLS) mellett a jóllét (WHO-WBI – Well-Being Index, magyar nyelvű validáció: Susánszky és mts., 2006) és az időskori életminőség (WHOQOL-OLD – Quality of Life, Tróznai és Kullmann, 2007; The WHOQOL-OLD module – manual, 2006) mérésére szolgáló kérdőíveket használtuk. Az időskori életminőséget mérő kérdőív hat alskála, témakör mentén méri fel a jóllétet időskorban: az érzékelési képesség, az autonómia, a múltbeli-jelenbeli-jövőben tervezett tevékenységek, a közösségben való részvétel, a halál és haldoklás továbbá az intimitás megélése.

    Eredmények
    Az eredmények alapján megállapítható, hogy mind az élettel való elégedettség, mind pedig a jóllét esetében hasonló erősségű és irányú kapcsolat van az idősödéssel kapcsolatos attitűdök kérdőív alskáláival, valamint az időskori életminőség kérdőív alskáláival. Magasabb fokú elégedettség, jóllét, autonómia, a közösségben való nagyobb mértékű részvétel esetén inkább növekedésként élik meg az idősödést, míg az egyre nagyobb mértékű érzékszervi hanyatlás és a haláltól-haldoklástól való nagyobb mértékű félelem esetén inkább veszteségként élik meg az idősödést. Ezen kívül a magasabb fokú autonómia, elfoglaltság, a közösségben való nagyobb mértékű részvétel magasabb fokú jólléttel és elégedettséggel jár együtt, illetve a haláltól-haldoklástól való nagyobb mértékű félelem esetében a legkisebb a jóllét és elégedettség érzése. A jóllét skálának az élettel való elégedettség skálával való magas korrelációja miatt ugyanakkor nem érdemes mindkét kérdőívet használni egy vizsgálaton belül.
    A két kutatás eredménye alapján a felhasznált három, szubjektív jóllétet mérő kérdőív közül elegendő egy használata az idősödés megélésének vizsgálatakor, míg az időskori életminőséget mérő kérdőív további szempontokkal tudja gazdagítani a vizsgálatokat az idősödéssel kapcsolatos attitűdöket mérő kérdőív mellett.

    A tanulmány rávilágít arra, hogy a megtett életút minősége, milyen lenyomatot képez időskori énünkben. Az öregedés folyamatának komplex bemutatásával árnyaltabb diagnózist lehet felállítani az időskor sokarcúságáról, ezáltal hatékonyabb prevenciós-, és gondozó tevékenységet lehet folytatni. A tanulmányban vázolt pozitív életfilozófia alkotóelemeinek megismerése és gyakorlatban való alkalmazása, hozzájárul az időskori mentális egyensúly fenntartásához.

    Gyakorlati relevanciák: A kutatás eredményei elsősorban gondozóintézetekben idősotthonokban és nyugdíjas klubokban hasznosíthatók. Hatékonyan segíthetik az idős emberek testi - lelki - szellemi gondozását és problémás élethelyzetük elfogadását. A tanulmány felhasználható az alap és középfokú andragógiai képzéseken és szaktanfolyamokon (szociális munkás, szakápoló, terapeuta).

  • Staff Training and Stress in Long Term Care Facilities Special Care Units for Alzheimer's Elders

    Objective:
    Special Care Units (SCU) in long term care health facilities are named to indicate "unique to diagnosis" or a level of care. The purpose of this study was to explore perceptions among caregivers and licensed nurses in selected nursing homes in Ohio and Pennsylvania and New York, as they pertained to the differences in care to Alzheimer's elders in SCUs. It examined the education and experience of staff and the satisfaction of this staff as it pertained to stress and wages.

    Background:
    Long-term caregivers often experience stress, resulting in "burn-out" as a consequence of limited training, levels of care required, cognitive decline of elders and family expectations. The caregivers, on SCUs, environmentally designed for the elders with cognitive decline, need specialized training in the physical and mental dimensions of the various forms of dementia, Alzheimer's type.

    Methods:
    The study used a qualitative research design with a survey questionnaire and one-on-one interviews with administrators and human resource directors. A pilot study of SCUs in Ohio and Pennsylvania and New York was initiated. The population was the employees of these SCU. The levels of employees questioned included: Executive director/Administrator, nurses, nurse aides, housekeepers and activities staff, laundry and social service workers. The procedure was standardized to enhance the reliability of the data. The respondents were notified in advance of the specific application of their answers and were afforded the opportunity to receive a monetary donation to the SCU of their respective facilities.
    An Eden Alternative Home, with a program of goals and missions characterizing enhancement of an elder's life, a home-like environment and family-centered staff and care, in a very rural area of Pennsylvania, was also engaged to contrast and compare the hypotheses of the study.

    Results:
    On the SCU, less than 5 percent of the staff had received training specific to the care of the Alzheimer's elders. In the Eden Home, 100 percent of the staff had received training specific to the care of the Alzheimer's elders. On the SCU, less than 18 percent were satisfied with the quality and quantity of specialized and extensive training. In the Eden Home, 100 percent of the staff was satisfied with their specific training. On the SCU, using a Leiken scale, more than 55 percent felt that they should receive higher wages. In the Eden home, the results were the same. However, the longevity of the employees was 13.5 years as opposed to only 3.25 years in the SCU in other homes.

    Conclusion:
    These findings suggest there is a need to examine, expand and intensify the training of all caregivers on a special unit for the elders afflicted with dementia, Alzheimer's type.

  • Postoperative rehabilitation after total knee joint replacement

    Objective:
    The pilot study objective was to evaluate the effect of comprehensive post-operative physiotherapeutic treatment on the selected physiotherapeutic parameters as well as patients’ quality of life after a total knee joint replacement surgery.

    Results:
    Thirty patients after total knee joint arthroplasty were enrolled into the pilot study. After completing the physiotherapeutic processes, we have observed among the patients who underwent a total knee replacement surgery, a significant increase in muscle strength as with the flexors as well as the extensors (p = 0.001), improvement in their range of motion in knee joint flexion, in comparison to its range before treatment (p = 0.001), knee pain reduction (p = 0.001) and post-operative swelling (p <0.001), respectively. After undergoing the physiotherapeutic procedures, we recorded a statistically significant improvement in the monitored physical components: dressing (p = 0.008) and toilet use (p = 0.001), transfer from bed to chair (p = 0.008), walking on flat surface (p = 0.001), climbing stairs (p = 0.001). Passing the physiotherapy significantly reduced the degree of dependence of operated patients (p = 0.001).

    Conclusion:
    Early post-operative physiotherapy treatment positively affects patients’ general condition. In addition, it improves muscle strength, range of motion of the operated joint, eliminates postoperative pain and reduces swelling that improves patients’ self-sufficiency. Standard physiotherapy nowadays and in the next few years will take a unique place in patients’ management after total knee joint replacement.

  • A geriátriai readaptáció szerepe a nem rehabilitálható idősek állapotjavításában

    A 65 éven felüli, többféle krónikus betegségben szenvedő idősek esetében a rehabilitáció lényegesen nehezebb, mint a fiatalabbaknál. Akut történések, vagy meglévő betegségek rosszabbodása esetén fontos szakmai kérdést jelent, hogy a páciens mennyire alkalmas rehabilitációra. A diagnózis, a prognózis és a rehabilitálhatóság komplex vizsgálata alapján a nem rehabilitálható egyének esetében elsődleges szempont, hogy önállóságuk minél tovább megmaradjon, ami együtt jár a jobb életminőséggel. Ezt segíti a geriátriai readaptáció, amelynek minél szélesebb körben való bevezetése és alkalmazása kulcsfontosságú az időseknél.

    Az idősek gondozása minden társadalom számára kihívást jelent. Számos kutatás látott már napvilágot az idősgondozással kapcsolatban, amelyek segítenek abban, hogy az idősgondozás minél hatékonyabban tudjon megvalósulni. Jelen tanulmány célja, hogy áttekintést nyújtson az etnikai kisebbségek közül a roma/cigány idősek gondozásához hazai, illetve nemzetközi trendek segítségével. Szempontrendszerünkben a célcsoport választása nem volt véletlen – a roma/cigány kisebbség több tényezőt tekintve is vulnerabilisnak tekinthető, mely az időskori gondozásra nézve veszélyeket rejt a gyakorlatban az egyenlőtlenségi viszonyokat tekintve. A tanulmányban az idősgondozás főbb fogalmain túl kitérünk a jogi környezet adta lehetőségekre és korlátokra, az idősgondozás és az egészségügy kapcsolatrendszerére, illetve az életminőség és jóllét alakulásának tényezőin túl olyan stratégiákra (formális és informális), melyek hazai és nemzetközi szinten is trendként rajzolódnak ki.

  • Social security and safety of older adults in Poland

    Poland’ population will be ageing at a fast rate in the coming decades. It is projected that in 2070 the Polish ratio between people aged 65 and over and those aged 15-64 years will be 62.6, the highest among EU-27 countries. Population ageing appeared in the public debate in Poland as a separate subject in the 1990s, following a negative natural population increase and the looming impact of the massive withdrawal of baby boomers from the labour market on the pension system. One of the reasons for older persons’ growing interest in retirement was pension system reforms planned by successive governments.

    The announcement of the year 2012 as the European Year for Active Ageing and Solidarity Between Generations (decision no. 940/2011/EU of the European Parliament and of the Council of 14 September 2011) contributed in Poland to the emergence of a senior policy from a social policy and initiated major legislative, institutional, and organisational changes at the national, regional and local levels of government. It also inspired the redefinition of measures used hitherto in line with the evolution in the perception of older people from social care recipients to active members of their communities entitled to education and economic, social, civic and political activity. In 2013, the Senior Policy Council was established as a consultative and advisory body to the Ministry of Labour and Social Policy, and then similar councils supporting regional and local authorities were organised.

    In order to encourage greater activity among seniors, a special governmental programme was created in December 2013, which grants funds on a competitive basis to projects concerning social activities, education, and intergenerational cooperation submitted by informal and formal groups of seniors.

    The national government’s key documents on senior policy, one for the period from 2014 to 2020 and the other spanning the years until 2030, are Resolution 238 of 24 Dec. 2013 by the Board of Ministers on the Adoption of Long-term Senior Policy in Poland for the Years 2015-2020 and Resolution 161 of 28 Oct. 2018 by the Board of Ministers on the adoption of Social Policy Towards the Older Persons 2030. Security-Participation-Solidarity. In 2015, the Polish Parliament passed the elderly people act, which requires institutions in charge of the well-being of older persons to monitor and report on their situation. The reports submitted by the institutions are used by the Ministry of Labour to compile and present an annual evaluation of the status of the older population in Poland.

    The regional governments’ senior policy is reflected in their social policy strategies. The strategies’ operational goals started to address needs specific to older people since 2002, focusing in particular on improving their quality of life, developing round-the-clock care services, at-home care services, and rehabilitation services, and on reducing social exclusion and marginalization of seniors.

    Social security and the safety of older adults are progressively improving in Poland, but the greatest progress has been made in the area of active ageing. Social care services for the elderly still require improvement, because the predominant family care model is inefficient in many ways due to:

    • limited financing of care services by public institutions,
    • the growing proportion of single elderly persons,
    • the increasing number of people aged 85+ (the so-called double population ageing),
    • social insurance disregarding long-term care to an elderly family member as an insurable risk,
    • a lack of legislation allowing employed people to seek a long-term leave to give care to an older family member,
    • the informal expectation that women who retire at the age of 60 years will take care of the oldest family members.

    While neither the scale nor the quality of home care services given to older persons is regularly surveyed in Poland, it can be presumed that the scale of care services is insufficient and that they excessively burden families with a member in need of care. Between 2010 and 2018, the number of persons aged 65+ increased in Poland by over 1.5 million, the number of the users of attendance services and specialised attendance services by 29,000 (from 99,000 to 128,000), and the number of residences in homes and facilities providing assistance to aged persons by 7,000 (from 20,000 to 27,100.)

  • Idősek az infokommunikációs térben – lehetőségek és kihívások

    Az idősek infokommunikációs technológiákkal történő interakciója pozitívan befolyásolhatja szellemi tevékenységüket és összességében jó hatással lehet a mentális egészségük serkentésére. Ez a mentális stimuláció javíthatja a memóriájukat is. Az IKT-t használó idősek úgy érzik, sokkal magabiztosabbak és kevésbé vannak elszigetelve a társadalomtól, megnöveltnek látják a társadalmi támogatást és társadalmi interakciót, jobb kognitív képességekkel rendelkeznek, fennmaradhat a függetlenség érzése a mindennapi életükben és körükben alacsonyabb szintű depressziót mérnek a kutatók. Az IKT-t megfelelő szinten alkalmazó személyek akár magasabb hozzáadott értéket is képviselhetnek közösségükben, mivel képesek átadni bölcsességüket és tudásukat a fiatalabb generációknak a fejlett technológián keresztül is. A fentiek következtében csökken a digitális szakadék, javulhat a személyes életminőség, a függetlenség és autonómia fenntartása a mobilitást is fokozza, kialakulhat és működhet a személyre szabott integrált szociális és egészségügyi ellátáshoz való hozzáférés, így a digitális kompetenciák színvonalas alkalmazása alapvetően, összességében pozitívan tud hatni az idősek saját életére és a hozzájuk tartozókéra is (Blazun, 2013).

    Jelenleg – főleg az elmúlt évtizedekhez képest – a kifejezetten idősebbek számára készült technológiai eszközök és megoldások felé mutatott igény, ennek következtében pedig ezek piacképessége is fellendülésben van. A gerontechnológia – melynek területéhez tartozik minden olyan okos szolgáltatás és technikai innováció, amely az idősek életminőségét javíthatja – segíthet a sikeres öregedésben és az önálló élet fenntartásában. A kapcsolati szolgáltatások erősíthetik a szocializációt és csökkenthetik az esetleges ellátási költségeket (Peterson & Prasad, 2011). 2020-ban az idősekkel kapcsolatos médiavisszhang minden eddiginél kiemelkedőbb volt a magasabb egészségi kockázat és a korona vírussal szembeni nagyobb veszélyeztetettség miatt (Köttl, Tatzer & Ayalon, 2021).

    Amikor az idősek az IKT lehetőségeket használják, számos kedvező hatással találkozhatnak. Digitális tudásuk fejlődésével csökkenhet a kirekesztődésük, ezzel együtt a magány érzete is, egészségi állapotuk folyamatos monitorozásával lassítható az egészségromlás, addig ismeretlen, új információkhoz tudnak hozzájutni, mely által fenntarthatják vagy növelhetik a mentális kondíciójukat. Kialakíthatják és menedzselhetik a családi és baráti kapcsolattartást, távolabb élő személyekkel tudják felvenni a kapcsolatot, illetve, ha mindezekben fiatalabb családtagjaik segítik őket, szorosabb családi köteléket és intergenerációs hasznot is hoz a folyamat (Bene, Móré & Zombory, 2020). Figyelmet kell fordítani arra ugyanakkor, hogy a pozitív tényezők mellett megjelenhetnek a negatív vonatkozások is. Ezek magában foglalhatják azokat az érzéseket, amelyek a személyes kontaktus hiánya miatt keletkeznek, olyan egészségi hatások erősödhetnek, mint a szem terhelése, a hosszabb ideig tartó ülés miatt rosszabbodó testtartás, internetes visszaélések célpontjai lehetnek az idősek, esetleg felmerülhet körükben is internet-és okoseszköz függőség.

    Előadásunkban az idősek és az IKT kapcsolatát vizsgáljuk a tanulás, a magányosság az egészségügy és egyéb, általánosabb területek vonatkozásában a nemzetközi szakirodalmon keresztül.

    The severe cases of COVID-19 in Europe. A comparative analysis using SHARE data base
    The paper focuses on the severe cases of COVID-19 in Europe and the predictors of these cases. The author used the secondary analysis of a large data base (SHARE) of persons aged 50 or more from the EU countries and the Switzerland. The data have been collected in the summer of 2020. In the first part the focus is on the theoretical starting point of the analysis. In the second part the main outputs are presented.
    How to convince the undecided - communication with the elderly and attitudes towards vaccination COVID-19 in Poland
    The pandemic has had its toll on the socio-economic life of most people. Governments and international organizations face new challenges in a situation of uncertainty. Social campaigns are designed to encourage as many people as possible to join the vaccination plan. Achieving population resilience requires a significant commitment of organizational, financial and communication resources.
    Older people constitute a priority group in the vaccination system in Poland. However, despite the measures taken, there remains a significant share of seniors who remain reluctant to vaccination. The government is taking further pro-turnout measures targeted at the elderly. The shortcomings of the existing strategies should be eliminated for the benefit of society.
    The aim is to summarize the research conducted in the field of the needs and possibilities of communication with the elderly, attitudes towards vaccinations and sources of influence on making purchasing decisions of seniors.
    Vaccination attitudes – international project in Central and Eastern Europe
    The beginnings of the project date back to 2020. The uncertainty that became commonplace in the times of the pandemic indicated new areas of previously unknown dependencies. Stress, uncertainty, attitudes towards the threat, protective behaviour, and soon also attitudes towards vaccination seemed the most important at that time. Examining the existing dependencies would allow a better understanding of the phenomenon of social responses to the pandemic and would support governmental and non-governmental, national and international actions that would be an important tool in the fight against the negative consequences of a spreading COVID-19.
    Scientists from Central and Eastern European countries representing different disciplines (sociology, economics, communication, statistics, psychology and psychiatry) joined forces in a common international project investigating the relationship between attitudes towards vaccines and the level of perceived mental health, uncertainty, socio-demographic variables and the level of religiosity. Core aims of the project were to analyse the model of predictors concerning demographic, psychological, and social factors influencing the intention to vaccinate against COVID-19 and to provide the information to prepare and implement more effective and efficient health messaging strategies and campaigns targeting groups reluctant to COVID-19 vaccination in Eastern and Central European countries.
    The project involved the development of a common research tool - a questionnaire, parallel research in four countries of Central and Eastern Europe (Poland, Romania, Slovenia, Hungary), more than 2000 respondents (unvaccined adults) and joint development of the results of the analysis, using the potential of all involved partners.
    The project is still developed to next stages of investigation. Results will be published in high quality journals in years 2022 and 2023.

    Survey on unvaccinated Hungarian elderly people during the COVID-19 pandemic
    During the pandemic, we performed a comparative study of the Hungarian unvaccinated people, focusing on psychological and demographic aspects in our questionnaire. The survey data collection lasted from 27 August to 13 October 2021. Participants were over 18 years of age who have not yet received any vaccination against the COVID-19 virus.
    The method used is an on-line questionnaire survey.
    The survey is not for political but for scientific purposes, and we hope that its results will contribute to a more professional management of the COVID-19 epidemic. The survey of Hungarian team was conducted in collaboration with Polish, Slovenian and Romanian university researchers.

    Well-being at the time of pandemic (happiness, health.)
    Depression, Anxiety and Stress Scale (DASS21)

    The presentation focuses on the most important aspects of the first analysis of the Hungarian results. The study involved 478 people: 113 men, 363 women, and 2 people identified as “other”. It is worth highlighting from the first processing of the data that a large number of unvaccinated people do not believe that they are at risk of getting the COVID-19 virus. In our sample 42% of the unvaccinated are very sure that the vaccine will not work, and 30.6% are afraid that they will become infected because of the vaccine. They do not usually feel safe after vaccinations, they are afraid of the undiscovered consequences, and they trust more in their natural immunity. 61.8% definitely do not want a vaccine for themselves, nor in the future. 67.3% want to wait to see how vaccines work in other people. During processing, we could identify the ones with a definite anti-vaccination attitude. They were also analyzed on the basis of their political affiliation, religiosity, and place of residence. Regarding the DASS-21 depression subscale, the young age group scored significantly higher than the elderly. According to the DASS-21 stress subscale, the value belonging to young people is significantly higher than that of older people. On the Vaccination Attitudes Examination (VAX) Scale’s concerns about financial profiteering subscale, seniors scored significantly higher. Interpreting our results in the context of parallel international studies is also part of our objectives. Our results provide an opportunity to serve important aspects for the communication strategies with unvaccinated elderly.

    Ahhoz, hogy az időskor jövőjéről képet alkothassunk, fel kell tárni az adott jelen időben és helyen az időskor előzményeit, amiben s ahogy az egyén, a csoport, a társadalmi réteg az időskort elérte, eléri, valamint a jelenben azokat a feltételezhető társadalmi, gazdasági átalakulásokat a jövőben, amelyekre a trendek mutatnak. Az időskorúak társadalmát heterogén társadalmi rétegek alkotják, amelyekben azonos, többnyire közel azonos, az időskori lét általánosítható egészségügyi, mentális, a családi közösségi kapcsolatokban, hasonló állapot változások történnek, folyamatok indulnak meg. Ugyanakkor az időskorúak összességét számos elválasztó létfeltételi, vagyoni, ideológiai, műveltségi állapot tagolja, amelyeket tovább bont a nemek eltérő társadalmi aktivitása, közösségekben való részvétele. Az utóbbi évtizedben számos kezdeményezés indult, illetve erősödött fel az azonosságok és különbségek feltárására, elemzésére, az időskorúak életminősége, társadalmi aktivitása fejlesztése, a káros folyamathatások perfektibilis kezelésére, azonban az átfogó, minden rétegre kitérő megoldások messze nem érik el a kívánatos szintet, nem biztosítják az időskorúak korunkban megkövetelhető létfeltételeit, társadalmi aktivitását.

    According to Botswana's Current statistics for 2021, people aged 60 and above would make up 8.9% of the population. As people age, they face a plethora of challenges; mental, legal, social, health, economic, environmental, and political, in addition to ageism and marginalization. Therefore, they need resilience to deal with these challenges that emanate from the aging process, the development of care needs, and the depletion of resources. However, in Africa, there is anecdotal evidence that some older people cannot cope with their lives, let alone carer responsibilities caused by HIV related death and other risk factors. For older people to flourish, they need resilience to achieve, endure, develop and sustain their health and well-being in the face of adversity. The degree of success and impact on their resilience is undocumented. The research will therefore determine the correlates with resilience and establish ways to curb the risk factors.

    Ageing society is a reality for many people in developing countries than in the past. Governments are facing major challenges in safeguarding the mental health of older people and health care systems to deal with this demographic shift. The mental health of older people is an important indicator of the level of their health-related quality of life.  The situation of older people in Africa, concerning their mental health and well-being, is a matter of growing attention among researchers and policymakers alike. There is a common perception in connection with the aged population reporting poor mental health status and a greater need for healthcare. However, there is few research on the disparities of older people relating to healthcare in Sub-Saharan Africa, especially in Botswana. There is a tendence of ever-increasing number and population proportions of older people representing both opportunities and challenges. Some of these challenges include the prevention and management of anxiety, depression, and somatization, which are the most common mental disorders in primary health care the world over. Previous research has shown that the three disorders are highly comorbid because of the need for prevention and mitigation of all three.

    Although rates of Covid-19 infection are lower for older adults, the mortality rates are higher than all other ages. While the health challenges are evident, the mental health effects of the pandemic are less evident. The term “compassionate ageism” has been used to describe protection of all older adults as vulnerable groups, however it overlooks individual factors that may mitigate risk and enhance mental health aspects of coping with the realities of the pandemic. Isolation and separation from families and once enjoyed activities, whether residing in their own homes or long-term care facilities, has taken an immeasurable physical and mental health toll on older adults. Safety and security for mental health as well as physical health must be considered in any strategy to try mitigate the effects on the quality of life of older adults during this critical time in our history.
